Who You Are:

- You have strong verbal and written communication skills, specifically involving documentation, progress notes, incident reports, and other related administrative tasks.

- You have the ability to multi-task and prioritize.
- You have proficiency in computer skills, including Microsoft 365 software, databases, electronic communication, and web-based applications.
Job Responsibilities:
- Communicate effectively, learn new skills readily, and build strong relationships with students, peers, families, and interdisciplinary teams.
- Collaborate across departments and with providers to support students’ holistic physical and emotional health.
- Work with RNs to monitor student wellness, perform routine and as‑needed health assessments, and respond to medical crises, including coordination with EMS.
- Support medical case management by scheduling appointments, administering medications, tracking vitals, and coordinating with guardians, providers, pharmacies, nutritionists, and food services.
- Provide health education directly to students and through Direct Support Professionals to promote healthy lifestyle habits.
- Participate in interdisciplinary teamwork across psychiatric, clinical, educational, and direct‑care domains.
- Demonstrate cultural competence, professionalism, and the ability to engage effectively with diverse individuals and communities.
- Mentor adolescents and young adults to build self‑advocacy, self‑determination, and healthy habits.
- Completes and maintains all required trainings, including TCIS.
- Transport students in a company vehicle to appointments may be required.

Position Requirements:
- Licensed Practical Nurse in NYS, current and in good standing
- A minimum of three (3) years’ experience as an LPN
- Valid driver's license with the ability to pass a DMV driving abstract screening required.
- Demonstrated ability to achieve desired results working collaboratively in a dynamic environment with multiple stakeholders
- Experience in program design, management, and implementation
- Due to the nature of work in supporting our students, all job description list the physical requirements for that position
- Our campus consists of some paved areas but primarily consists of grass, gravel paths and parking lots. Additionally, many areas of our building are fully accessible, however, as a historic building some areas are only accessible by stairs.
- You must be able to ambulate around the campus, including hills, stairs, grass and gravel.
- Student Facing Positions:
- Must be able to react and move quickly during student crisis, both for the safety of the student and for the staff.
- Must be able to run across the campus and throughout the school to catch/monitor an eloping student, and/or to remove yourself from an unsafe situation.
- You must be able to pass TCIS (Therapeutic Crisis Intervention Training for Schools).
- You must be able to bend, stretch, sit, stand, crouch or kneel for sustained periods of time if implementing a hold.
